We can typically get a new patient in the same day in an emergency situation and within 24-48 hours in a non-emergency situation. Contact us today to schedule an appointment.

After downloading the New Patient Form, be sure to have: 

• Current insurance information, Medicare and/or Medicaid card
• If HMO, referral from primary care physician
• Co-pay or self-pay payment at time of service
• List of any current medications
• Primary care physician name and phone number
• Records from previous mental health providers

All co-pays, coinsurance, and deductible amounts are due at the time of service. Dean Aman, LPCMH LLC reserves the right to cancel an appointment if payments has not been made.

There is a $40 service fee for returned checks or declined credit cards.

24 hours notice is required to cancel an appointment. A missed appointment fee will be assessed if 24 hours notice is not given.

If you are unable to complete the forms yourself, a charge will be incurred that is not covered by your insurance. The charge for completing forms including but not limited to, leave of absence, FMLA, and Short/Long-Term Disability or medical forms is $25 per form.

Yes. Medication changes are only made during an office or telepsych appointment.
